A leaky bladder isn’t just an embarrassing inconvenience

KUALA LUMPUR: Many people laugh off the occasional “oops” moment when coughing, sneezing or laughing too hard causes urine leakage.

Others assume bladder-control problems are simply a part of ageing, and opt to quietly stock up on pads or avoid social gatherings altogether.

Health experts caution, however, that urinary incontinence – also known as “leaky bladder” – is not something people should ignore or suffer through in silence.

According to consultant urologist Dr Warren Lo, urinary incontinence is not a disease in and of itself, but a symptom caused by problems involving the muscles, nerves or structure of the urinary tract.

In some cases, it could point to more serious underlying conditions such as diabetes, kidney disease, neurological disorders, or even cancer.

“Every case of urine leakage has an underlying cause,” he said. “Delaying medical assessment could lead to late diagnosis of a more serious illness.”

Urinary incontinence can take several forms. Some people experience leakage when pressure is placed on the bladder during coughing, laughing, sneezing or exercise – a condition known as stress incontinence.

Others may suddenly feel an urgent need to urinate and struggle to reach the toilet in time, often linked to an overactive bladder.

There are also cases where the bladder does not empty fully, causing urine to dribble continuously.

Lo said bladder-control problems can affect anyone regardless of age or gender, although certain groups may face higher risks. Pregnant women and mothers who have gone through multiple deliveries, for example, may develop weakened pelvic-floor muscles that contribute to urine leakage.

Meanwhile, neurological conditions such as Parkinson’s disease, spinal-cord injuries and stroke may interfere with the body’s ability to control bladder function properly.

Lifestyle habits can also play a role. Frequent consumption of caffeinated, carbonated or alcoholic drinks may irritate the bladder, while men with enlarged prostates or bladder stones can develop symptoms linked to overactive bladder conditions.

Despite how common the issue is, embarrassment often stops people from seeking help. Some avoid social activities altogether as they fear accidents in public, while others quietly spend large amounts on adult diapers and pads without addressing the root cause.

Left untreated, persistent urinary leakage may also increase the risk of recurrent urinary tract infections (UTIs), skin irritation and emotional distress. In severe cases, infections linked to urinary problems can spread into the bloodstream and become life-threatening.

The good news, Lo highlighted, is that urinary incontinence is often manageable – and in many cases treatable.

Treatment depends on the type and severity of the condition. Pelvic-floor exercises such as Kegels may help strengthen muscles and reduce leakage, while reducing caffeine and carbonated drink intake can sometimes ease symptoms linked to overactive bladder.

Modern treatment options now also include medications, Botox injections into the bladder and devices sometimes referred to as “bladder pacemakers”, which help regulate bladder function.

For Lo, the biggest challenge remains changing public perception. Too many people, he said, still assume bladder leakage is just something they have to live with.

But while the topic may feel uncomfortable to discuss, experts stress that persistent bladder control problems are not a normal part of ageing – and should not simply be brushed aside.
